I am a business intelligence analyst at a hospital and I work with clinical data every day. Doctor Mike made a huge point that most people don't realize, that is appropriateness of care (ex antibiotic stewardship) has a huge impact on cost. Luckily, many insurance providers are moving to a population health focused program called 'Value based care' which puts the financial risk on Hospitals instead of insurance providers for appropriateness of care. They basically say we will pay x for this type of patient, it is your job to manage their care. What has come out of this for us on the hospital side is a ton of process improvement work to reduce waste and manage the whole patients health through primary care to reduce our financial risk (and it is directly tied to physician compensation). This benefits both the patient and the hospital as care becomes more controlled and appropriate and incentives the system to properly care for the whole patients health instead of being focused on fee for service. We are also a non-for-profit so those hospitals do exist! Awesome stuff Mike!

I'm happy for Czech medical care, you can wait ages and meet nasty employee, but your insurance company pay all. (almost all, but you almost never pay the whole price for prescripted medications (except Ibubrofen, Aspirin, common nose spray and all this stuff...)).

The cost of health care is so high is because people don't pay their bills. My son was hit by a car, which broke his neck and jaw. People constantly told us, "Just pay them $10 a month. They won't sue you." So, if everyone does that, the hospitals have to get their money somewhere. They then have to raise prices for everyone else/insurance companies. I've also heard from several people who refuse to pay for insurance and just hope for the best. So when a catastrophe happens, they just declare bankruptcy or try the $10-a-month deal. It is basic math as to why costs are going up. The only answer is universal healthcare where no one can opt out.

Also, think Adam was saying that hospitals should pay their administration less, but even if that happened, even if you had a magic wand and started paying admin $25,000 a year, the problem of unpaid bills would still be there.

I see both sides. However, I have been the victim of this outrageous pricing. My daughter broke her pinky in 6th grade and I had lost my job and insurance because of the housing crisis and even had to pack up and move back to my home town at the time so it was already a stressful time. I took her to the ER and paid almost $700 for the visit, xrays, and temp cast. They sent me to the ortho clinic. The dr walked in, looked at the X-ray sent from the hospital, didn’t say shit to me or my daughter at all, told the aide to wrap her hand a certain way... and walked his ass out. He didn’t even LOOK at my daughter let alone talk to her or explain anything to use. Guess what the bill I got from them was on top of the $700 I had already paid... $1300 fucking dollars. Not even joking. $28 in supplies, $200 office visit... and over a grand for the “doctors expertise”. I fought with them for a long time over this bullshit and after paying the office visit said I wouldn’t pay any more of the bill. They admitted they charged me the same as if the dr had actually done surgery even though he didn’t, let alone interact with us at all. Ugh this was years ago and I still get pissed off thinking about it. I get why they have to overcharge to battle with insurance companies but taking it out on people like me who had every intention of paying FAIR prices for an emergency isn’t the way to practice business. The $1000 I paid for a simple cast was more than fair.

I'm even more impressed that you did it in a non combative manner, and with respect. Awesome video and awesome information. This has left me with a lot to think about and encouraged me to learn more.... Now I need to figure out how. Last note, I am so glad that you were so insistent that when it comes to medical knowledge it is always best to go with the most recent information. Great videos please keep it up.

I gotta take Adams side here - the profit motive is the problem. I wanna add tho, feminists have been criticizing the overuse of mammograms since the 60s, Germaine Greer wrote about it in The Female Eunuch which was published in 1970. We've known for a long time that mammograms are being over used, but the medical guidelines frankly ignored this fact for many years. That doesn't inspire confidence. This conversation is missing the political dimension wherein medicine has been used to control women's bodies through things like overused mammograms and other tests, medicalised childbirth, not to mention involuntary sterilization that was done to specific groups of women such as Native/Aboriginal women and disabled women - and is still being done today.

As someone easily scared by anything health related, I agree that attempting to talk about mammograms with comedy was not effective. There is no point in scaring people away from tests that they need, and if I did not take action to properly educate myself, I would be easily convinced to NOT get one, which is dangerous. Adam's responses were so misinterpretive it makes me wonder if they were purposefully misinterpretive so as to further the agenda.

His show is often overs simplified and intentionally favors "edginess" over accurate representation of facts. Used to watch the show, but I stopped when I realized its flaws. All in all, if you want to learn about a topic, don't watch some watered down comedy show and do your own research.

I like Adam, and the respect the way he researches his topics, but he is good at spinning and not so good at taking criticism. I think he isn't so focused on the fact you agree with his facts, just differ on the presentation. He feels justified, which is fine, but he isn't looking for the conversation.

I definitely agree with Mike on this topic, especially about the Mammogram segment scaring women away from getting one. Watching the previous video where Mike reviewed the episode, watching the Mammogram segment scared me from getting one even before Mike commented on it about how it could possibly scare women away. I've always been scared of when the day comes for when I have to get one, and that put me off even more.

Adam needs to chill. He's a comedian, but he isn't a doctor and that makes a difference in this situation. Dr. Mike has had a lot more practice in explaining a medical diagnosis, so he knows how a patient might react to something like the information Adam presented (which Mike generally agreed with in terms of accuracy). I had a mammogram earlier this year, just before my 30th birthday because I found a lump and I have a family history of breast cancer. If I had seen that video before I had gotten the mammogram, I might not have pushed the hospital to get me scheduled ASAP after my doctor had said I should get one. The mammogram was definitely uncomfortable, but you know what was even more uncomfortable? Not knowing if I was going to have to go through chemo or radiation therapy or have a mastectomy (Mom had to do the latter two). Adam can take a hike, I'd rather have Dr. Mike explain this subject to the masses.

I really used to enjoy Adam ruins everything...but the reactions I see from him (not just here) when information in his show is challenged is unbecoming, to be the most polite I can. I LOVE how you, Dr.Mike, are able to be so even and positive in light of the borderline condescending tone presented in those tweets.
